The player model seems to clip out of places pretty frequently (both the model and camera). This is probably a mix of incorrect camera placement and unfit rigidbody calculation option settings

Likely has to do with rigidbody collision

Player can clip through moving wall with a certain setup, possibly caused by the camera affecting the player position
The setup for anyone who wants to test it out involves standing in the middle of the hallway, looking at the left wall, pointing your camera all the way down, and then looking up and to the right a bit.
