When a question is derived from another question, keep track of its parent. The parent can be identified by the hash of the question body.
We can add metadata as comments. E.g. meta.parent: <hash value>.

We can also add other types of metadata, e.g. how well the students did on this particular version of the question.
We can also use the metadata to only use the latest version of the question.
