IMO it's a bit risky that during use of the form builder you don't see what template you are editing and also that there is no indication of how often this template is used.
I think most site administrators will just go to a page, edit the used template and not release that the template might also be used on other pages.
Therefore I suggest adding both the name and usage of the template to the Form Builder:

Obviously it should be "5 Page(s)"

IMO it would also be good to show this on the form settings.
