Dead clients with callbacks are not dropped #2

This was specifically noticed on ShellProvider, but it's probably true on all APIs. If the client process has a binder disconnect, we will continue attempting to send callbacks even though they will never succeed. For shell in particular, the command will continue being executed when it should be dropped.
