- Line 560: Wrap the onUpstreamOpen lifecycle callback in an error boundary; if
it throws, close and settle the connection. Protect onUpstreamClose from
escaping its event or timer callback, swallowing callback errors after teardown.

- don't dial when `onUpstreamConnect` closes the peer itself - log errors thrown by `onUpstreamOpen`/`onUpstreamClose` instead of crashing - report the underlying upstream error (not the event) and no synthetic code - give each peer its own target `URL` copy

…allbacks crossws 0.4.16 (h3js/crossws#219) reports proxied connections with `onUpstreamConnect`/`onUpstreamClose`, exactly once each, so the bridge no longer wraps the proxy's `open`/`close`/`error` hooks. The target is synchronous when the runner is ready (dialed in the same tick), and the manager refuses a dial to a runner swapped out meanwhile.
Resolves #218
Adds three optional lifecycle callbacks to
createWebSocketProxy(), so callers can track proxied connections (for example, counting connections per upstream for drained reloads) without wrapping the returned hooks.Changes from the proposal in the issue
onUpstreamConnect, and it fires when the target resolves, not when the upstream opens. That way it pairs 1:1 with the close callback: connect timeouts and unreachable upstreams never reach "open", but they still need to be settled for accounting. A separateonUpstreamOpencovers "the handshake completed".onCloseis renamedonUpstreamClose. This keeps it apart from the proxy's ownclosehook. The callback also gets asource: "client" | "upstream" | "proxy"field that says who ended the connection."proxy"covers connect timeout, client idle timeout, buffer limit and setup failure.urlis the resolvedURLobject, not a string.Guarantees
onUpstreamClosefires exactly once for everyonUpstreamConnect, on every teardown path: client close or error, upstream close or error, connect timeout, idle timeout, buffer limit, and setup failure.onUpstreamConnectthrows, the peer is closed with1011andonUpstreamClosereceives the thrown error.onUpstreamCloseruns only after the proxy has finished tearing down, so a callback that throws can't leave the upstream or the peer half-closed.Tests and docs
