ToU Hotfix

• Update Terms of Use checkbox language

• Adds the "General Information Layer" (GIL), and incorporates its style
  • Major overhaul of style and content available sans authentication
  • GIL style incorporated in much of the other UI (eg login, registration, etc)
• Enables P3P
• Enables Symptom Tracker
• Top banner styling: logo moved to center, patient actions moved to left (more like GIL).
• GIL style left slide-out menu added to patient's dashboard (home page)

We need the pre-release version of Flask-User for integration with oauth.

Addressed several issues in the contact form, including handling unicode

characters in names, subjects, body.  Default to logged in user email
and name if available.  Pick up server value rather than hardcode to
prod.

Set CONTACT_SENDTO_EMAIL config variable to destination address.
